§ 98-113. Application for permit; required information.

  • To obtain a permit applicants shall supply to the chief of police the following information: Name, social security number, and date of birth of applicant, and if a corporation, the names, social security numbers and dates of birth of all officers and directors who shall be designated as to their office; address of applicant, and, in addition, if a corporation, the address of all officers and directors; names of employees, social security numbers and dates of birth and their addresses; criminal convictions, other than traffic offenses of applicant, officers, directors, and employees; street address of place from which business is to be conducted, and the location where items described in section 98-112 shall be kept during the time period set forth in section 98-114; a recent photograph of applicant, officers, directors, and employees; and the days of the week and hours of the day during which business will be conducted. A willful providing of false information shall constitute a violation of this article.
